Both the Player Registration Form and Medical Release form must be filled out and submitted at registration in order for a player to be able to partiipcate in Little League.

There are two CORI forms, both of which are required for all adults that have repeated interactions with players. Please fill out and submit both forms when filling out the volunteer/coaches application.
